Session Description: Hospital discharge can be a challenging and difficult process for children with medical complexity. Problems that arise soon after discharge can contribute to hospital readmission. Nurse-led post discharge home visits are able to identify these issues and provide collaborative solutions.
Primary Presenter Bio: Sarah has over thirty years as a pediatric nurse at Boston Children's Hospital(BCH). For the past seven years she has been the Clinical Nurse Specialist for the Complex Care Service at BCH which is a multidisciplinary team that cares for medically complex patients. She is the advanced practice nurse who has conducted the post discharge nurse-led home visits.

Presenter 2 Bio: Jayne has been the nursing director for inpatient medicine at Boston Children's Hospital for 30 years. She has worked creatively to enhance care for patients. This project was her brainchild conceived to understand then challenges related to discharge of children with medical complexity. She id the nursing member of a research team focused on improving discharge practices.
